HMLC is proud to offer its devotees and surrounding community the Moderna COVID-19 vaccine COMPLETELY FREE of charge.

*We cannot provide 2nd Dose to those who already have their 1st Dose elsewhere*

 

1st Dose on Thursday, April 8th, 5 to 9 PM

2nd Dose on Thursday, May 6th, 5 to 9 PM

 

After registration, and before the vaccination day, you will get a "Follow-up" email for additional information. This will save time on the registration day

Eligible categories:

 

1. Age: 65 years and above

2. Age: 18 to 64 years with underlying conditions: PROOF of condition required and can be any form submitted to us stating an underlying condition in Category 1B+ (e.g. cancer, chronic kidney disease, Chronic Obstructive Pulmonary Disease (COPD), diabetes, heart conditions, immunosuppressed states from a solid organ transplant, pulmonary disease, and sickle cell disease)

3. Caregivers: Must be a paid position

4. Manufacturing plant workers

5. Grocery store workers

6. Front-line essential workers

7. IT Employees essential workers

8. Restaurant workers

9. Category 1C eligible
